School, camp & sports physicals

Before your child participates in school or other activities, a physical exam is often required to make sure your child can take part in the activity without hurting themselves or others.

Novant Health Arbor Pediatrics regularly completes school, camp and sports physicals. Often, these exams can be completed and any necessary forms filled out during your child's annual well-child exam. If you need a form filled out, please let us know when you make your appointment, and please bring the form with you as we will not have access to it in our office.

If your child is up-to-date on well-child exams, we can ordinarily complete needed forms between appointments as well. Please allow at least three days for form completion.

If you have necessary forms filled out during your child's annual visit, there is not normally an extra charge for form completion. However, if your child needs a separate physical exam to have a form completed, or needs tests not included in an annual exam, there might be an extra charge for those services. If you are concerned about coverage, please ask your insurer or someone in our office.
